American Diabetes Association: Providing Diabetes Support in Your Community

In the United States, racial/ethnic minority populations have higher rates of diabetes with significantly higher risk of complications compared to non-Hispanic Whites. Diabetes support programs provide ongoing support to people with diabetes to maintain and expand the skills, knowledge, and lifestyle changes they gain from diabetes self-management education and support (DSMES). Diabetes support programs have been shown to reduce complications and improve overall health and quality of life but, unfortunately, are not readily available in health disparate communities.

During this webinar, presenters will provide an overview of the American Diabetes Association’s Standards of Medical Care in Diabetes, focusing on elements most relevant to the diabetes care team, including community health workers (CHWs). Using a case-based approach, the presenters will then discuss how CHWs have successfully implemented diabetes support programs in health disparate areas integrating culturally relevant education and resources. Special attention will be placed on the CHW’s role as part of the diabetes care team and how health centers can utilize these best-practice tools and resources to create community-clinical linkages.
