Self-Management: Road Map Towards Blood Pressure Control

The National Center for Farmworker Health (NCFH) in collaboration with the American Heart Association invites you to join us in this 60-minute webinar on hypertension which is the leading modifiable risk factor for heart disease and stroke; and Self-measured blood pressure (SMBP) enables physicians to better diagnose and manage hypertension, and helps patients take an active role in controlling their blood pressure. The presenters will review the steps and resources available to help participants build an SMBP program and support their patients in self-managing their heart health.
